Diabetes support

Health First Medical Group Diabetes Self-Management Education Program

Health First Medical Group diabetes self-management education program provides the highest level of education to diabetes patients so they are able to better care for themselves through proper nutrition, medication, exercise, monitoring and prevention of complications.

The program is recognized by the American Diabetes Association and the National Committee for Quality Assurance Diabetes Physician Recognition Program. Education is provided by clinical Certified Diabetes Educators including a Registered Dietitian and a Registered Nurse.

Health First Medical Group Diabetes Education Program

Educational Classes--comprehensive program of six to eight hours total with strong emphasis on nutrition and meal planning.

There are three options for attending the classes offered in Melbourne, Merritt Island and Viera:

  • A series of two consecutive classes from 9 a.m. to 1 p.m. with varying days offered on a weekly basis
  • A Friday class offered monthly
  • A Saturday class offered every other month.

Individual Visit--an individual consultation with the nurse diabetes educator to address further questions and concerns after completing diabetes classes.

Refresher Classes--once the comprehensive program is completed, you may attend a 2-hour annual update class every year.


Medicare and Health First

Diabetes self-management education is a covered benefit.

Commercial Insurance Carriers

Many commercial carriers provide benefits for diabetes education. Your insurance company is contacted regarding coverage before scheduling you for education. Most should expect to have the usual office visit co-payment.

Program Registration

If you are interested in participating in the diabetes education program, please speak with your physician regarding a referral to the program. In addition, you can contact our Endocrinology department at 321.728.6012 option 5 for more information on obtaining a referral.

Diabetes medical nutrition therapy

Individual and group appointments are available with a registered dietitian to address individual needs related to meal plans and other health-related issues that affect nutrition. Physician referral is required for this service.
